Steps to Follow

Requesting an Internship:

Your first step is to complete the Internship Intake Form. To open the form below click on the arrow in the upper right hand corner beside the form - this will take you to Google Forms. Enter your information and click SUBMIT to send your information to your Work Based Learning Department

You will receive an email in return to schedule an appointment with the CDC. Please check your emails regularly.
