Before You Go

There are several forms that must be completed prior to reporting to the company sponsor site.

First, complete a CCS CTE Internship Commitment form. Work-based Learning Agreements are developed among the school, the student, and the participating business and outline the rules and responsibilities of all parties involved in the internship.

Next a Parental Permission Agreement form should be completed and placed on file with the Career Development Coordinator.
